• Therapy progress notes, nursing notes, any other pertinent notes

• Current Medication List

• CARE Assessment, if needing LTC

• Advances Directives (DPOA, Living Will, DNR, Etc.)

• Copies of insurance cards, ID cards, and social security card

What's needed for a hospice referral?

• Face Sheet

• Order for Hospice

• Health & Physical Progress notes from Physician

• Medication List

• 2 weeks of nurses notes

• DPOA

• DNR If one on file

• Labs
